What is polyvagal theory in simple language?

Polyvagal theory was developed by Dr. Stephen Porges, shown above, as a way to understand how the autonomic nervous system responds to safety, connection, stress, trauma, and threat.

The autonomic nervous system controls many important automatic bodily functions, including heart rate, breathing, digestion, blood flow, and the stress response. You do not have to think about these systems for them to work. They are constantly adjusting in the background, trying to help you survive, adapt, and recover.

One of the easiest ways to picture this is through the Polyvagal Ladder, which describes three main nervous system states:

  1. Ventral vagal activation: connection and safety, where the body has more access to rest, digestion, sleep, repair, and social connection
  2. Sympathetic activation: protection mode, where the body mobilizes for fight-or-flight and may show signs such as racing thoughts, muscle tension, poor sleep, digestive urgency, irritability, or increased pain sensitivity
  3. Dorsal vagal activation: survival mode, where the body conserves energy and may move into fatigue, low motivation, brain fog, numbness, withdrawal, or shutdown

These states are survival responses. Your body is always asking, Am I safe enough to heal, or do I need to protect myself?

That question is not always answered by logic. You may know you are safe, but your body may still be responding as if it is not. This is especially common after chronic pain, trauma, prolonged stress, illness, repeated medical frustration, or years of feeling like your body cannot be trusted.

The nervous system does not reset through logic alone. It learns through safety, repetition, consistency, and experience.

The Polyvagal Ladder

Most people move up and down the Polyvagal Ladder throughout the day. That is normal. The concern is when the nervous system gets stuck in protection or shutdown so often that the body never feels settled long enough to repair.

At the top of the ladder is ventral vagal activation, the state most associated with connection and safety. In this state, the body has more access to rest, digestion, sleep, and repair. You may feel more present, emotionally steady, and able to respond instead of react.

In the middle of the ladder is sympathetic activation, or protection mode. This is the fight-or-flight response. It can be helpful in a true emergency because the body increases energy, changes blood flow, sharpens attention, and prepares you to act.

But when sympathetic activation becomes the body’s everyday setting, it can create problems.

Signs may include:

  • Anxiety or racing thoughts
  • Jaw, neck, or shoulder tension
  • Poor sleep
  • Digestive urgency or digestive disruption
  • Increased pain sensitivity
  • Pain flares with inflammation
  • Irritability or feeling constantly on edge
  • Difficulty settling after stress

At the bottom of the ladder is dorsal vagal activation, or shutdown and survival. This can happen when the nervous system feels that mobilizing is no longer working, or when the body has been under strain for too long.

Signs may include:

  • Fatigue
  • Low motivation
  • Depression or emotional flatness
  • Brain fog
  • Numbness or disconnection
  • Withdrawal from people or activities
  • Feeling heavy, stuck, or depleted
  • Chronic pain without obvious visible inflammation

This state is often misunderstood. A person in dorsal shutdown may look unmotivated from the outside, but their body may actually be conserving energy because it no longer feels sufficiently resourced to engage.

This matters because a shutdown nervous system does not always need deep relaxation first. Sometimes it needs gentle activation, connection, rhythm, warmth, light movement, or small doses of support before it can move upward.

Why chronic pain can keep the body in a stress response

When pain continues for a long time, the body can begin to organize life around it.

Movement may start to feel risky. Sleep may become disrupted. Muscles may stay tense. Inflammation may linger. A person may begin scanning every sensation, wondering whether it is a warning sign, a flare, or proof that something is getting worse.

That constant uncertainty is stressful.

Over time, the nervous system may begin treating normal life as a threat. A car ride, a workday, a family event, a new symptom, or even the thought of pain returning can trigger a protective response.

When stress remains unresolved, the body may adapt by increasing vigilance, tightening muscles, changing breathing patterns, altering digestion, shifting blood flow, and keeping the system ready for the next problem.

That pattern can interfere with healing because repair requires resources. The body needs energy for tissue repair, cellular communication, immune balance, sleep, digestion, and recovery. When the nervous system is spending too much energy on protection, less may be available for restoration.
